Medication Reconciliation

The patient which I had interviewed had taken the medications as followed: Fluticasone Proprionate, Citalopram, Naproxen Sodium, Dicyclomine, Ondansetron, Topiramate, and Albuterol. Safety concerns that were raised entailed the usage of alcohol or cannabis with Topiramate, Ondansetron, and Citalopram due to the fact that these are all central nervous system depressants, which could lead to serious consequences. The risk associated with Naproxen Sodium is gastrointestinal bleeding, thus, there is a high importance in eating food while administering said medication. There is also a risk of bleeding in general with this medication thus, this patient should utilize a soft bristle toothbrush to prevent bleeding of the gums. With Topiramate as well as Citalopram there is high importance on safe sex patient education, as the patient is sexually active and is of reproductive age, these medications increase the risk for fetal abnormalities if the patient becomes pregnant. Safety concerning Dicyclomine and Ondansetron is urine retention thus, there was high importance on ensuring the patient is adequately hydrated and understands how to properly monitor input and output to ensure that renal function is adequate. There is also a risk this patient may develop oral thrush from corticosteroid usage, Fluticasone Propionate, thus the patient was educated on the importance of rinsing their mouth out with water post usage of this steroid. The patient had known the purpose for all medications which they were prescribed (Asthma, Anxiety, TMJ inflammation, Irritable Bowel Syndrome, Persistent Nausea, Migraines, and Asthma again). Two medications had been utilized for the same diagnosis however, one (Albuterol) is utilized in emergent cases. There was however a drug to drug interaction between Citalopram and Ondansetron which the patient was unaware of, this drug to drug interaction entails serotonin syndrome which can be extraordinarily harmful to said patient. Thus, the patient had been informed that there was a need to monitor for any potential adverse effects which may occur with the utilization of these two prescriptions: tachycardia, sweating, flushing, vomiting, tremors, nausea, and muscle rigidity.
